Role Summary:
As a Patient Care Coordinator, you will be responsible for ensuring seamless coordination of patient care services at our healthcare facility in Port Harcourt, Nigeria. You will serve as a liaison between patients, healthcare providers, and other support staff to facilitate efficient and effective patient care delivery. Responsibilities:
- Coordinate patient appointments, referrals, and follow-up care.
- Serve as the primary point of contact for patients, addressing their inquiries and concerns.
- Collaborate with healthcare providers to develop and implement patient care plans.
- Maintain accurate patient records and documentation.
- Educate patients about their treatment plans, medications, and available resources.
- Monitor and evaluate patient satisfaction and quality of care.
Requirements:
- Bachelor’s degree in Healthcare Administration, Nursing, or related field (preferred).
- Proven experience in a healthcare or customer service role.
- Strong communication and interpersonal skills.
- Excellent organizational and multitasking abilities.
- Knowledge of medical terminology and healthcare processes.
