Role Summary:
As a Clinical Pharmacist at our client’s healthcare facility, you will play a key role in optimizing medication therapy and promoting safe and effective medication use. You will collaborate with healthcare providers, participate in interdisciplinary care teams, and provide clinical pharmacy services to improve patient outcomes.
Responsibilities:
- Review medication orders and patient profiles to identify drug-related problems, including drug interactions, allergies, and contraindications.
- Collaborate with physicians, nurses, and other healthcare professionals to develop and implement individualized medication therapy plans for patients.
- Monitor patients’ responses to medication therapy, including therapeutic and adverse effects, and make recommendations for therapy adjustments as needed.
- Provide drug information and education to patients, caregivers, and healthcare providers on medication use, dosing, administration, and potential side effects.
- Conduct medication reconciliation and discharge counseling for patients transitioning between care settings, ensuring continuity of care and medication safety.
- Participate in pharmacy and therapeutics committee meetings to review and evaluate medication formulary, policies, and protocols.
- Stay updated on the latest evidence-based guidelines and clinical research in pharmacotherapy to provide optimal patient care.
Requirements:
- Bachelor’s or Doctor of Pharmacy degree from an accredited institution.
- Valid pharmacy license to practice in Nigeria.
- Proven experience working as a Clinical Pharmacist, preferably in a hospital or clinical setting.
- Strong clinical knowledge and understanding of pharmacotherapy principles and practices.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a multidisciplinary team environment.
